Your Therapist

JASON MODAFARI, DPT

Doctor of Physical Therapy, Tampa, FL

Jason's clinical focus includes baseball rehab, throwing injuries, and high-performance return to sport. His strength and conditioning background shapes sessions that can include barbell work, plyometrics, sport-specific drills, and graduated return-to-play progressions.

Manual therapy and dry needling are available when they serve the plan, then the work returns to strength, power, and sport exposure. Jason builds each program around the demands of the athlete, from UCL reconstruction and return to throwing through full-body rehab for active adults.

The Facility

BUILT FOR REHAB AND ATHLETIC DEVELOPMENT.

We operate inside Driveline Baseball, a training facility built for athlete development. Throwing rehab uses radar, arm-health testing, mounds, cages, and the strength floor. Athletes ready for performance motion capture, live at-bats, or pitch design can transition to Driveline's development staff under the same roof.

Indoor Mounds & Cages

Full-size Driveline pitching mounds and batting cages for return-to-throw progressions and position-specific work.

Turf Training Area

Open turf for sprinting, agility, cutting, and sport-specific return-to-play work across sports.

Strength & Racks

Full gym with racks, platforms, and dumbbells. Whether you are rehabbing an ACL or building back from shoulder surgery, rehab is training here.

PULSE, ArmCare & FlexPro Grip

Driveline arm speed tracking, workload monitoring, arm health screening, and FlexPro Grip for forearm and grip strength loading.

PlyoCare & Weighted Balls

Complete Driveline weighted ball and arm care setup for progressive throwing loading protocols.

Manual Therapy & Dry Needling

Hands-on soft tissue work, joint mobilization, and trigger point dry needling integrated directly into training-based sessions.

The Process

HOW THIS WORKS

01

EVALUATE

Sixty-minute one-on-one assessment. Range of motion, strength, movement quality, throwing mechanics, and your full injury and training history. If you have imaging, we review it. If we need more data, we use the Driveline lab. You leave with a clear diagnosis and a plan.

02

TREAT & TRAIN

Every session combines physical therapy with training. Manual therapy, dry needling, strength work, throwing progression, and data collection all happen in one facility. Standard follow-up visits are 45 minutes, one-on-one, with Jason.

03

PROGRESS WITH DATA

Every milestone is objective. Strength benchmarks, range-of-motion targets, velocity thresholds, and workload tolerance guide when the program progresses.

04

RETURN TO COMPETITION

Full competitive throwing, game-speed demands, and performance testing come before clearance. We stay involved through the transition back to full competition.

Pricing Model

CASH-BASED. ON PURPOSE.

We don't bill insurance because the care we deliver doesn't fit an insurance model. No visit caps. No handoffs to aides. No 15-minute time slots. Superbills provided for out-of-network reimbursement.
