PRIVACY POLICY

INFORMATION WE COLLECT

• Scheduling information. When you use the Site to book an appointment, we receive the clinic, appointment type, date and time, email address, telephone number, and phone-country selection you enter. The Site sends the booking information to PtEverywhere. PtEverywhere account registration takes place on PtEverywhere's website and is governed by its own privacy terms.
• Care and cost estimate information. The Site may store the clinician, location, service selections, estimated duration, price calculations, treatment preferences, payment preference, and any selected Cherry payment example in your browser while you build or schedule with an estimate. If you ask us to email the estimate, we also collect the email address you provide.
• Advertising lead information. If you submit a lead form on Facebook, Instagram, or another advertising platform, we may receive your name, email address, telephone number, preferred clinic or provider, broad service interest, contact preference, and the consent choices shown on that form. The exact fields depend on the form you submit.
• Waitlist and email information. If you request Arizona opening updates, we collect your email address, consent record, subscription status, and unsubscribe or suppression status.
• Device and network information. Website hosting and security systems process ordinary request data such as the page requested, date and time, IP address, browser or user-agent information, referrer, and security or error records.
• Website-use information. We may use Vercel Web Analytics to measure aggregate page views, referral sources, general device and browser type, approximate region, and selected actions such as a booking-link or phone-link click. Query strings and page fragments are removed before an analytics event is sent. We do not attach form contents, appointment details, names, email addresses, or telephone numbers to those events.
• Browser storage. The Site stores temporary estimate information and delivery status in session storage. It may also remember interface preferences, such as dismissing an announcement. The embedded Cherry financing widget creates anonymous browser identifiers and sends widget-use and device information to Cherry and its service providers, including Segment.

WHERE INFORMATION COMES FROM

We receive information directly from you, from your browser and device, from PtEverywhere when a booking request is processed, and from Meta or another platform when you choose to submit one of our lead forms there.

HOW WE USE INFORMATION

• Respond to the request you submitted and route it to the correct clinic.
• Show availability and submit appointment requests to PtEverywhere.
• Create, deliver, and discuss care and cost planning estimates.
• Provide requested waitlist or clinic-opening updates.
• Operate, secure, maintain, and troubleshoot the Site.
• Prevent duplicate bookings, fraud, misuse, and automated abuse.
• Measure general Site use and the source of Site visits.
• Meet legal duties and protect our rights, users, and systems.

A consultation request or lead-form submission does not automatically place you on a general marketing list. We will use the information for additional marketing only when the form or another sign-up process gives you that choice and you agree.

META ADS AND LEAD FORMS

Athletic Potential may run general advertisements on Facebook and Instagram. We do not place Meta Pixel on the Site, use Meta Conversions API, upload Site visitor data to Meta, or use Site browsing and scheduling activity to build Meta custom audiences.

Meta still processes information about activity that takes place on Facebook or Instagram. If you submit an Athletic Potential lead form inside a Meta product, Meta collects the form information on its platform and sends a copy to us. We use that copy to respond to your request, route you to the requested clinic, and provide the follow-up described in the form. We do not sell Meta lead information or use it for an unrelated purpose.

Please keep diagnoses, symptoms, surgery details, medical records, and other clinical information out of advertising lead forms. Clinical information belongs in the patient process identified by your clinic. Meta's handling of information on its services is described in the Meta Privacy Policy.

WHO RECEIVES INFORMATION

We disclose only the information needed for the work described in this Policy. Recipients may include:

• Athletic Potential businesses and personnel. Information is routed to the clinic or provider connected with your request.
• PtEverywhere. Account registration, scheduling, appointment confirmation, patient administration, and clinical records.
• Vercel. Website hosting, delivery, security, technical operations, and Web Analytics when enabled.
• Upstash or Vercel KV. Short-lived, hashed booking safeguards used for rate limits, duplicate-request protection, and temporary slot locks. These records do not contain your name, email address, telephone number, or clinical information.
• Resend and Google Workspace. Delivery and receipt of estimate emails and permitted contact messages.
• Kit. Arizona opening emails for people who separately request those updates.
• Cherry and its service providers. Embedded financing tools, payment examples, widget operation, and any financing application you choose to start with Cherry. Cherry applies its own terms and privacy policy to its services.
• Legal, safety, and transaction recipients. Government authorities, advisers, or a successor to the business when disclosure is required by law or reasonably needed to protect rights, safety, Site security, or complete a business transaction.

We do not sell personal information. We do not disclose Site visitor information to Meta for cross-context behavioral advertising.

HEALTH INFORMATION AND URGENT REQUESTS

The public Site and advertising lead forms are not patient portals. A page you visit, a service you select, or an appointment you request may still reveal that you are interested in health care. Our separate Consumer Health Data Privacy Policy explains Washington consumer rights for that information.

Do not use a public form, advertising lead form, or ordinary email for urgent or emergency concerns. Call 911 or seek appropriate emergency care when needed.

RETENTION

We keep personal information for as long as needed to respond to requests, provide services, maintain security, meet legal duties, resolve disputes, and preserve ordinary business records and backups. Retention periods differ by record and service provider.

Browser session storage normally clears when the browser session ends. Booking safeguard records expire automatically, generally within minutes and no later than 24 hours. An email subscription remains active until you unsubscribe, request deletion, or the list closes. We may retain a limited suppression record so we can honor an unsubscribe request.

SECURITY

We use administrative and technical safeguards intended to protect personal information. Internet transmission and electronic storage always carry some risk. Use the patient system identified by your clinic when clinical information is requested.

YOUR CHOICES

You may ask to access, correct, or delete personal information that we control. We may verify your identity and authority before acting on a request. Available rights and exceptions depend on where you live and the type of information involved.

Marketing subscribers may use the unsubscribe link in a marketing email. You can also adjust advertising choices inside Facebook or Instagram through Meta's account settings. Those settings control Meta's platform and do not change a request that you already submitted to Athletic Potential.

CHILDREN

The Site is directed to adults. A parent or legal guardian should submit requests concerning a minor. We do not knowingly collect personal information directly from a child under 13 through advertising lead forms.

CHANGES TO THIS POLICY

We may revise this Policy when the Site, our services, or legal duties change. The revised Policy will appear here with a new effective date. We will provide another notice or request consent when the law requires it.
