PRIVACY POLICY

This Privacy Policy describes how personal information is collected, used, retained, and disclosed through athleticpotentialpt.com (the “Site”). The Site is operated for Athletic Potential Performance Physical Therapy LLC in Washington and Athletic Potential LLC in Florida. Together, these entities are referred to as “Athletic Potential,” “we,” “us,” or “our.”
This Policy applies to the public Site. It does not govern clinical records maintained through our patient systems, information covered by a separate patient privacy notice, or information you provide directly to an outside service.
THE SITE IS NOT A PATIENT PORTAL
The callback form is intended only to request contact from Athletic Potential. Do not submit information about an injury, diagnosis, surgery, symptoms, imaging, medication, treatment, or other clinical matters through the form or by ordinary email. The form is not monitored for urgent or emergency requests.
Clinical information should be provided only through the patient process we identify for you. PT Everywhere is our separate scheduling and clinical platform. If you use a PT Everywhere link, you leave the Site and provide information directly through that platform.
PERSONAL INFORMATION WE COLLECT
- Callback information. Your name, who the request is for, broad sport or activity context, baseball or softball role when applicable, preferred location, state for remote services, broad service interest, applicable delivery or treatment preference, what you want to cover, desired start timeframe, preferred callback method and time, email address or telephone number, and acknowledgment of the payment-model notice.
- Email subscription information. Your email address, consent record, subscription status, and unsubscribe or suppression status if you request Arizona opening updates.
- Device and network information. Information created when a browser requests the Site, which may include the requested page, date and time, IP address, browser or user-agent information, referrer, and security or error records.
- Aggregate website-usage information. Vercel Web Analytics measures page views, referral sources, general device and browser type, and approximate region. Query strings and page fragments are removed before an analytics event is sent. Analytics events do not include callback-form or email-list contents.
- Local browser preferences. The Site stores an announcement-dismissal value in local storage on your device.
SOURCES OF INFORMATION
We collect information directly from you when you submit a callback request or join an email list. We also receive technical information automatically from your browser, device, and network when you request a page or submit a form.
PURPOSES OF PROCESSING
We use personal information for the following purposes:
- To route and respond to a callback request.
- To confirm that the payment-model notice was presented and acknowledged.
- To send Arizona opening updates after a separate marketing opt-in.
- To operate, secure, maintain, and troubleshoot the Site.
- To understand aggregate Site use and evaluate referral and outreach activity.
- To prevent fraud, misuse, and automated abuse.
- To comply with law and protect our legal rights or the safety of others.
Callback requests are not added to a marketing list. We do not use Site information to make decisions about employment, insurance, credit, or housing.
DISCLOSURES OF PERSONAL INFORMATION
We disclose personal information only for the purposes described in this Policy and as permitted by law. Recipients may include:
- Athletic Potential entities and personnel. Callback information is routed to the entity or provider responsible for the location or service you request.
- Vercel. Website hosting, delivery, security, technical operations, and aggregate web analytics.
- Resend. Transmission of callback requests to the clinic mailbox.
- Google Workspace. Receipt and handling of callback requests in the clinic mailbox.
- Kit. Arizona opening emails for people who have provided separate marketing consent. Kit does not receive callback requests.
- Legal and safety recipients. Government authorities, advisers, or other persons when disclosure is required by law or is reasonably necessary to protect rights, safety, or Site security.
- Transaction recipients. A successor or other party involved in a proposed or completed reorganization, financing, merger, acquisition, or transfer of business assets, subject to applicable law.
We do not sell personal information. We do not disclose personal information for cross-context behavioral advertising or targeted advertising.
ANALYTICS, ADVERTISING, AND BROWSER STORAGE
The Site uses Vercel Web Analytics to measure aggregate page views and selected actions, such as a callback request, contact-link click, directions-link click, or resource download. We do not attach names, email addresses, telephone numbers, form contents, or clinical information to these events. Vercel states that Web Analytics is cookie-free and does not store a visitor's IP address or use a persistent identifier to recognize a visitor across sites.
The Site does not load Google Analytics, Meta Pixel, or another advertising tracker. Google Maps and YouTube content are provided as external links rather than embedded services. The announcement-dismissal preference described above remains in your browser's local storage. More information appears in our Cookie Policy.
RETENTION
We retain personal information for as long as reasonably necessary to fulfill the purpose for which it was collected, respond to requests, maintain Site security, meet legal obligations, resolve disputes, and maintain ordinary business records and backups. Retention periods may differ by record and service provider.
An email subscription remains active until you unsubscribe, request deletion, or the list is closed. We may retain a limited suppression record after an unsubscribe request so that we can honor the request.
INFORMATION SECURITY
We maintain administrative and technical safeguards designed to protect personal information against unauthorized access, use, alteration, and disclosure. No method of internet transmission or electronic storage is completely secure. Please use the patient process we provide if clinical information is requested.
YOUR PRIVACY CHOICES
You may request access to, correction of, or deletion of personal information that we control. We may take reasonable steps to verify your identity and authority before acting on a request. Rights and exceptions differ by jurisdiction.
Marketing subscribers may use the unsubscribe link in any marketing email. An unsubscribe request does not prevent communications needed to respond to a separate request or provide a service.

CHILDREN'S PRIVACY
The Site is directed to adults and is not intended for independent use by children under 13. A parent or legal guardian should submit a callback request concerning a minor. Clinical information about a minor should be provided only through the patient process identified by Athletic Potential.
THIRD-PARTY SERVICES
The Site contains links to scheduling, mapping, video, and other third-party services. Those services operate under their own terms and privacy practices. We are not responsible for a third party's handling of information you provide directly to that third party.
CHANGES TO THIS POLICY
We may revise this Policy as our practices or legal obligations change. The revised Policy will be posted on this page with a new effective date. We will provide any additional notice or obtain consent when required by law.
