PRIVACY POLICY

THE WEBSITE IS NOT A PATIENT PORTAL

Our public form only asks us to contact you. It asks for your first name, preferred location, preferred callback method, and the email address or phone number needed to respond. You may also choose one broad service area. If you select return-to-throw or strength and conditioning, the form asks whether you prefer in-person, remote, or hybrid delivery. Tampa visitors who select manual therapy and recovery may choose a broad treatment option. The form requires confirmation that you understand our cash-based, out-of-network payment model and that nonclinical services are not eligible for insurance reimbursement. Please do not put injury, diagnosis, surgery, symptom, imaging, medication, or treatment information into the form or an ordinary email. The form is not monitored for urgent or emergency needs.

PT Everywhere is our patient scheduling and clinical system. If you choose a booking link, you leave this website and provide information directly to PT Everywhere under its terms and our clinical privacy practices.

INFORMATION THIS SITE HANDLES

• Callback requests: first name, preferred location, callback method, an email address or phone number, an optional broad service interest, a conditional delivery or Tampa treatment preference, and confirmation that the payment-model notice was acknowledged.
• Arizona opening emails: an email address and the record that the subscriber checked the Arizona marketing consent box.
• Basic hosting data: the requested page, date and time, IP address, browser or user agent, referring page when supplied, and security or error information created as part of serving the site.
• A local preference: whether this browser dismissed the site announcement. This stays in the browser's local storage.

We receive the form information directly from you. Hosting data comes from your browser and network when you request a page.

HOW WE USE IT

• To route and respond to your request and confirm that the payment model was shown before the callback.
• To send Arizona opening emails only after the separate opt-in box is checked.
• To deliver the site, prevent abuse, diagnose failures, and keep it secure.
• To comply with law and protect legal rights when necessary.

A callback request is not placed on a marketing list. We do not use public website form information to make decisions about employment, insurance, credit, or housing.

SERVICE PROVIDERS AND OTHER RECIPIENTS

The site uses a small number of providers for specific jobs:

• Vercel hosts the website and processes the technical request and security data needed to serve it.
• Resend delivers callback requests to our clinic mailbox. The message contains only the callback fields and acknowledgment listed above. Resend is not used to receive clinical information.
• Google Workspace is the clinic mailbox that receives the callback request. We have a Business Associate Agreement with Google, although this website form is still designed to exclude medical information.
• Kit receives only the email address of a person who explicitly asks for Arizona opening emails. Kit does not receive callback requests.

We may also disclose information when required by law or when reasonably necessary to protect safety, security, or legal rights. We do not sell personal information, and we do not disclose it for cross-site targeted advertising.

NO ADVERTISING OR ANALYTICS TRACKERS

The site does not load Google Analytics, Meta Pixel, Vercel Web Analytics, or another advertising tracker. Google Maps and YouTube do not load automatically. Their sites receive a request only if you click a directions or video link and leave this site. See our Cookie Policy for the browser storage used here.

RETENTION

Callback requests are kept only as long as reasonably needed to respond, manage the request, handle security or legal issues, and maintain ordinary backups. Resend's standard service currently retains email and delivery logs for 30 days while an account is active. Hosting logs are kept according to the Vercel plan and security settings in use.

An Arizona subscriber's email remains in Kit until the person unsubscribes, asks us to delete it, or the list is closed, subject to limited backup and legal retention. Unsubscribing stops future marketing email even if a suppression record must remain to honor that choice.

SECURITY

We limit the public form fields, reject unexpected fields, rate-limit submissions, keep provider keys on the server, and avoid logging form contents. The site also blocks third-party frames and tracker connections through its security policy. No internet transmission or storage system is guaranteed to be completely secure, so medical details should go only through the patient workflow we provide.

YOUR CHOICES AND REQUESTS

You may ask to access, correct, or delete personal information we control. Arizona email subscribers can use the unsubscribe link in any marketing email. Depending on where you live, additional rights may apply.

MINORS

We work with athletes who are minors, but this public website does not ask a child to submit medical details. A parent or legal guardian should make a callback request for a child and should provide clinical information only through the patient workflow.

CHANGES AND CONTACT

We will revise the effective date when this policy changes. If a change requires consent, we will ask before the new collection or use begins.
