Four ways to work with us.

Pick the path that matches the length of your recovery and how you train. Baseball athletes with serious injuries can keep PT, strength, throwing, and facility access together. Remote care can begin during injury rehab or at the return-to-throw stage. Other athletes can choose a six-week hybrid plan or book individual visits as needed.

Rehab Advantage Plan

For baseball athletes rehabbing inside Driveline after surgery or a serious injury, with at least 12 weeks ahead. Weekly PT, strength programming, return-to-throw management, and daily gym access are included.

PT visits, strength programming, and daily gym access are included. Missed sessions with notice bank for make-up after the term. Custom return-to-throw programming is added when you are ready.

HSA and FSA cards are accepted for eligible expenses. Eligibility and reimbursement depend on the plan and service. Superbills may be available for eligible PT visits. Program fees are not PT encounters.

Before You Enroll

PLAN POLICIES.

The terms depend on the plan. Read the section that matches the way you plan to work with us.

REHAB ADVANTAGE

  • Committed plans run for 12 weekly payments.
  • Cancel during the term and you pay the difference between the committed and Flex rates for care already delivered.
  • After 12 weeks, your rate stays locked for that episode of care. You may change tiers or cancel.
  • Sessions missed with notice can bank up to three weeks of your tier. No-shows do not bank.

HYBRID & REMOTE

  • The full six-week plan price is owed, including when you use an installment schedule.
  • Unused services are non-refundable and convert to account credit.
  • The plan ends after six weeks. There is no recurring plan to cancel.
  • A renewed Hybrid block begins with another 75-minute evaluation and retest.

BILLING & INSURANCE

  • HSA and FSA cards are accepted for eligible expenses.
  • Superbills may be available for eligible PT visits.
  • Program fees are not PT visits and do not appear on a superbill.
  • Clinical telehealth is matched to the provider licensed for the patient location.

WHY WE DON'T BILL INSURANCE.

Insurance decides how long your visit runs, how often you're seen, and when you're done, usually before anyone has looked at you. That model produces twenty-minute visits shared across three patients and a discharge date set by a benefit cap, not by whether you can actually go back and compete.

Cash-pay means the plan is built around your injury and your season instead. You get the whole session, one-on-one, with a DPT. We provide superbills so you can submit for out-of-network reimbursement. Some athletes get part of it back, and we give you everything you need to try.
